For the Engagement of Community Based Organization to provide TB/HIV services to Damien Foundation.

DAMIEN FOUNDATION (DFB) is a non-profit humanitarian medical organization affiliated with Damien Foundation Belgium (DFB) and is assisting the Federal Government of Nigeria with the National Tuberculosis and Leprosy Control Programme (NTBLCP) in Oyo, Osun, Ondo, Ogun and Lagos States. As Sub Recipient (SR) to IHVN on TBHIV project under Global Fund Grant, DFB has been vested with the responsibility of implementing Active TBHIV case finding programme in the aforementioned States. In view of this, DFB requires the services of Community Based Organization (CBO) to provide the services as detailed under the scope of work below:

DFB will respond to questions it receives prior to the due date for the submission of the EOI.

 

Submission of Expression of Interest

 

  1. All required documents must be submitted via the e-mail below on or before the due date and time or to:

Late prequalification will not be considered or accepted.

 

 

 

 

 

 

 

TECHNICAL REQUIREMENT

 

·         Evidence of registration with Corporate Affair Commission (CAC).

·         Evidence of registration with TB Network

·         Organization’s Profile which must include your experience with TBHIV related works/services

·         Financial credibility.

  
 

 

 Locations:

Oyo, Osun, Ondo, Ogun and Lagos states. (Kindly indicate your preferred state in the EOI).

 

The location delivery of products/services will be dependent on the venue for the activities within the coverage areas of the grant as indicated in the background above.

 

 

Language:

The Expression of Interest, as well as all correspondence and documents therein, shall be in English.

 

Due Date and Time for Submission of Expression of Interest

  • Prequalification documents must be received by DFB no later than the due date and time as shown above
  • Any document received by DFB after the due date and time for submission will be rejected.

 

Evaluation of EOI:

EOI will be evaluated based on the following criteria:

  • Overall responsiveness of the EOI
  • Financial Credibility
  • Positive feedback on reference check

 

Confidentiality:

Information submitted in response to this EOI will be held in confidence.
